The application below is to be used for applying to be a vendor for our 3rd Annual Christmas Extravaganza to be held on 11/20/10.  Application and registration fee does not guarantee you as a vendor.  We try our best to not have "duplicate vendors".  If we are unable to accept you as a vendor any fees and/or donations will be returned.  Please fill out form completely and read information at end.  Any questions feel free to contact us at info@walkonfoundation.com.
